Larger funding gap in statutory health insurance: Ministry expects €18.8 billion deficit in 2027

The increase of €3.5 billion jeopardizes the planned savings buffer. Instead of a small buffer, a new gap of €2.5 billion would remain if no additional measures are taken. The black-red coalition is thus under pressure to push a coordinated savings package through parliament before the summer break.

The Bundestag will address a corresponding savings package, which the cabinet has initiated, this Friday. Chancellor Friedrich Merz (CDU) has set the guideline for the parliamentary deliberations that the savings buffer must not become smaller.

Planned savings measures in detail

Health Minister Nina Warken (CDU) is planning relief for the health insurance funds amounting to €16.3 billion in 2027. Her concept provides for spending caps for medical practices, hospitals, and the pharmaceutical industry. In addition, higher co-payments for medications and restrictions on the contribution-free co-insurance of spouses are planned.

The goal of the entire package is to prevent further increases in supplementary contributions in the statutory health insurance system. Given the significantly higher spending in the first quarter of 2026, this undertaking is likely to prove more ambitious than originally planned.

Criticism from the healthcare sector

The healthcare sector is already responding with sharp protests against the planned cuts. Associations of statutory health insurance physicians, hospital representatives, and pharmaceutical associations had warned in recent weeks of supply bottlenecks and a weakening of the care structure should the savings package enter into force in the form presented.

The black-red coalition of CDU/CSU and SPD must now demonstrate whether it can enforce the Chancellor's guideline in the Bundestag. Voices from the governing factions had recently indicated that individual points of the package would still need to be negotiated. The coming days are considered decisive for the success of the undertaking.

Open questions and next steps

It remains unclear to what extent the planned savings measures will ultimately actually deliver the €16.3 billion in relief. Should the gap turn out to be larger in the end, a further increase in supplementary contributions for insured members would be likely — a scenario that the coalition is precisely seeking to avoid with the savings package.

The Ministry also pointed out that the revised figures are preliminary. A final estimate for 2027 is expected in the autumn, based on the half-year figures available by then. Until then, the assumptions could still change.
