Outbreak Meets Vaccination Gaps

The World Health Organization (WHO) and the U.S. health authority CDC had already pointed out in earlier reports the risk that declining vaccination rates in several U.S. states could favor the re-emergence of measles. The American Academy of Pediatrics (AAP) continues to recommend the two-dose MMR vaccination in childhood as the most effective protection against infection and possible complications.

Particularly affected, according to local health departments, are counties with lower vaccination rates. In some school districts, quarantine measures have been imposed on unvaccinated students after contact persons were identified. The incubation period of up to two weeks also makes it difficult to quickly contain chains of infection.

At the national level, the debate over vaccination policy has intensified. Robert F. Kennedy Jr. has long stood as a political actor for a vaccination policy oriented more strongly toward individual freedom of choice and has repeatedly expressed criticism of individual vaccines. Federal agencies such as the CDC, by contrast, emphasize the scientific consensus on the safety and effectiveness of the MMR vaccine.

Authorities in Pennsylvania announced that they would deploy mobile vaccination teams to particularly affected counties and expand information campaigns in several languages. Family practices have also been called upon to actively approach patients in order to catch up on outstanding vaccinations. Hospitals are preparing in parallel for possible additional hospitalized cases.

Physicians warn that measles must not – as is sometimes assumed – be regarded as a harmless childhood illness. Complications such as pneumonia, middle ear infections, and in rare cases a fatal brain inflammation (SSPE) can occur even years after the actual infection. Pregnant women and immunocompromised individuals are considered particularly at risk.

For the coming weeks, health authorities expect a further rise in case numbers, as school has resumed in several states. Schools and daycare centers are considered places with an elevated risk of transmission, which is why the authorities want to inform parents there about the importance of vaccination. Health departments are also observing the development with growing concern beyond the region.

The case illustrates the connection between vaccination rates and the occurrence of preventable infectious diseases. Experts see the current outbreak as a warning signal that the decades-long successes in combating measles in the United States cannot be taken for granted.
