Rare Ebola Strain Outbreak in DRC Raises Concerns Over Delayed Detection and U.S. Response

The virus, identified as the Bundibugyo strain, is genetically distinct from more common Ebola variants, with a sequence about 30% different. This divergence complicated early detection, as initial tests were designed for more typical Ebola strains. "Some of the initial testing that was done didn't pick up this Bundibugyo virus, since those tests were designed for more common versions of Ebola," a source noted.

Cases are concentrated in Ituri province, a conflict-ridden mining region, where humanitarian programs—now severely underfunded—once served as informal disease surveillance networks.

U.S. Response and Challenges

The U.S. is evacuating an infected American aid worker and six others considered high-risk to Germany for monitoring and treatment. The CDC is also surging staff to the DRC, including a senior technical coordinator, beyond its existing team of about 25 in the country.

However, the U.S. response has been hampered by recent cuts. USAID's DRC mission was shuttered last year, and CDC funding and staffing have been reduced over the past 18 months. Humanitarian funding for Congo dropped from over $900 million in the last year of the Biden administration to $179 million under the Trump administration—an 80% decrease.

The World Health Organization declared the outbreak an international public health emergency just two days after its announcement. Experts warn that delayed detection has given the virus a head start. "This outbreak has a lot of momentum," one observer said.

"My immediate impression was that this is an extraordinarily large number of deaths and suspected cases that was being reported in what was supposed to be a new outbreak," a source remarked. Another added, "My immediate instinct was that this has been ongoing for a couple of weeks and has taken some time to identify. That sent off alarm bells in my mind."

With no approved vaccines or treatments for the Bundibugyo strain, international health agencies are scrambling to contain the outbreak. The CDC and other organizations are mobilizing staff and supplies, but experts caution that the global response infrastructure is weaker than in previous years.
