"My immediate impression was that this is an extraordinarily large number of deaths and suspected cases that was being reported in what was supposed to be a new outbreak," said Boghuma Titanji, an infectious disease physician at Emory University. Initial reports indicated 246 suspected cases and 65 deaths, alarming experts who noted the unusually high figures for a newly detected outbreak.

Challenges in Detection

The outbreak involves a rare strain of Ebola called Bundibugyo, which has a genetic sequence about 30% different from more common Ebola virus species. "Some of the initial testing that was done didn't pick up this Bundibugyo virus," Titanji explained, noting that standard tests were designed for more familiar strains.

Jeremy Konyndyk, president of Refugees International and former director of USAID's Office of U.S. Foreign Disaster Assistance, pointed to broader systemic issues: "That disease detection surveillance architecture has been badly weakened." He cited cuts to humanitarian funding, which dropped from over $900 million in the last year of the Biden administration to $179 million under the Trump administration.

U.S. and Global Response

The U.S. Centers for Disease Control and Prevention (CDC) confirmed it only learned of the outbreak on May 14, the day before it was publicly announced. CDC Ebola response incident manager Satish Pillai stated, "Anything that the country office and the Ministry [of Health] is requesting for support, we will be providing." The agency has deployed additional technical and field experts to the DRC beyond its existing 25 staff members.

However, Konyndyk warned that reduced U.S. engagement with the WHO and shrinking budgets have hampered global readiness. "The whole international response architecture is much weaker than it was a few years ago," he said.

Future Concerns

With the outbreak already widespread, containment efforts face significant hurdles. "This outbreak has a lot of momentum," Konyndyk noted, emphasizing the need for rapid case identification and isolation. The WHO has declared the situation a "public health emergency of international concern," urging coordinated action to prevent further escalation.

Spanish Prime Minister Pedro Sánchez framed the crisis as a global security issue at the WHO's annual meeting, stating, "Investing in global health is investing in security for our people." Yet, with funding cuts and strained resources, the path forward remains uncertain.
