WHO warns: Ebola outbreak in eastern Congo could surpass the 2014 West Africa epidemic

According to the Congolese health authorities, the outbreak continues to be concentrated in the province of Ituri in the east of the country. This region has been the scene of armed conflicts for years, which significantly complicates the work of health teams. Aid workers report that entire villages are temporarily cut off from medical care because roads are impassable or attacks are being carried out on health facilities. The combination of violence and a highly contagious viral disease makes containment particularly difficult, according to experts.

Background: Conflict region as a hotspot

In view of this situation, Tedros Adhanom Ghebreyesus addressed the international community directly in a statement. He explained that the current outbreak has the potential to surpass the worst Ebola epidemic in recent history if the current trend continues. The WHO has sent additional specialists and laboratory capacity to the region, but these measures alone are not enough to stop the spread.

In response to the aggravated situation, the Congolese government, in coordination with the WHO, has expanded emergency measures. These include mobile testing stations that can analyze blood samples in remote areas within a few hours. International partners such as Doctors Without Borders and the International Red Cross are supporting the establishment of treatment centers where the sick are to be isolated and provided with medical care. At the same time, awareness campaigns are underway to inform the population about symptoms, hygiene, and the handling of the deceased.

International aid and vaccines

The international community is watching the developments with growing concern. Following the experience of the West African epidemic, in which over 11,000 people died, the WHO had significantly expanded its capacity to respond to virus outbreaks in recent years. However, in eastern Congo, these structures are reaching their limits because the security situation, the size of the affected area, and mistrust of external aid workers are hampering effective containment.

Another focus of concern is the question of vaccines. The rVSV-ZEBOV vaccine developed during the West African outbreak is in principle available, but the logistics for ring vaccinations – that is, the targeted immunization of contacts and their surroundings – are complicated in conflict zones. Many aid workers must operate under the escort of security forces, which takes time and resources.

Risk of cross-border spread

At the same time, concern is growing about cross-border spread. The provinces of Ituri and North Kivu border Uganda, Rwanda, and South Sudan. According to health experts, an uncontrolled spread would have not only humanitarian but also economic consequences for the entire region, as trade routes could be disrupted and travel warnings issued.

The WHO emphasizes that containment remains possible provided the financing of aid measures is secured and the security situation allows aid workers to operate unhindered. Ghebreyesus called on governments and donors to honor their commitments and not to slacken in view of the dynamics of the outbreak.

Outlook: WHO relies on enhanced surveillance

For the coming weeks, the WHO announced increased reporting in order to be able to identify and communicate potential trends at an early stage. At the same time, negotiations are underway with neighboring countries on cross-border surveillance teams that are to identify suspected cases before they cross the border.
