The SPÖ state premiers are opposing the health reform that has been negotiated between the federal government, states, municipalities, and social insurance providers for a year, and they are warning of a wave of hospital closures in their states.

The negotiations on a fundamental reorganization of the healthcare system, which have been ongoing for around a year, are threatening to fail due to the resistance of the SPÖ state premiers. Carinthia's State Premier Daniel Fellner sharply criticized the reform plans on Facebook and spoke of a "Schließungsorgie" regarding hospitals. He ranted: "Nein zu diesem Kahlschlag". Vorarlberg's State Premier Markus Wallner and Burgenland's State Premier Hans Peter Doskozil also took opposing positions, as shown by a placard bearing the slogan "Hände weg von unseren Spitälern".
