Ludwig demands direct billing for guest patients from abroad

To justify his position, Ludwig referred to the city's calculations: deducting persons from Vienna who are treated in other countries and a lump-sum amount from the financial equalization scheme, 610 million euros in costs remained last year, it is complained. This remaining sum is the share that Vienna actually bears for guest patients from abroad without having so far received full compensation.

The demand for direct billing is not new; similar attempts in earlier rounds of negotiations with the federal government and the states have, according to Ludwig, so far been without lasting success. Ludwig sees direct billing as a mechanism that could balance existing imbalances between high-performing hospital locations and smaller federal states.

The cost burden primarily affects the city's central and specialty hospitals, which perform supralocal care functions. These include facilities such as the Spital Speising, as well as other Viennese clinics that take in patients from inside and outside Austria for complex procedures.

In the financial equalization between the federal government, states, and municipalities, the handling of cross-border healthcare has been a contentious issue for years. Ludwig argues that a clear direct billing rule must complement the existing lump-sum mechanisms in order to reflect the actual treatment services of a city like Vienna.

Representatives of other federal states, including Lower Austria around St. Pölten, have in the past discussed both the advantages and disadvantages of such models. While high-performing locations would benefit, direct billing could bring additional administrative steps in smaller states.

Political assessment and outlook

The federal government's response to Vienna's renewed initiative remained open at the time of the dispatch via APA. Ludwig had recently also pointed, toward neighboring regions such as Burgenland, to the need for a uniform regulation.

Politically, Vienna's demand is carried by the SPÖ, which wants to bring the issue into the ongoing talks on the upcoming financial equalization period. Ludwig had already addressed the issue in previous rounds of negotiations and sees it as an essential contribution to the financial stability of Vienna's healthcare system.

Regardless of the specific regulation, Ludwig said Vienna wants to continue treating patients from abroad. However, the city sees itself obligated to shape the framework conditions so that the high upfront expenditures of Vienna's hospitals are also financially compensated.

In the coming weeks, observers expect further signals from the Ministry of Health and from the states on whether and in what form direct billing could be included in the next financial equalization. Ludwig announced that he would stick to the cause.
